The burden of chronic disease is a constant challenge for FQHCs in Maine. During this one-day workshop, we will hear from regional content experts from the NE QIN/QIO & HealthCentric Advisors about promising options available to health centers to build and advance team-based Type 2 Diabetes management programs.

Topics will include strategies for clinicians to engage with patients around lifestyle change, reimbursement for chronic disease services such as self-management programming and care management, and integration with Annual Wellness visit planning.
