Hear from Jaeson Fournier, CEO of the CommUnityCare Health Centers network in Texas and former CEO of the Federally Qualified Health Center Urban Health Network (FUHN), a health center coalition-led ACO in the Twin Cities area. FUHN brought together a group of health centers in a Medicaid ACO, built a robust data analytics infrastructure with the aid of a health services data management firm, created valuable delivery system improvements and generated millions of dollars in shared savings.
Jaeson will share the origin story of FUHN, lessons learned around health center-led ACOs/IPAs and value-based payment arrangements he has been part of in both Minnesota and Texas. Based on his experience, he will also share insights on how health centers can use value-based payment and care to advance population health in their communities.
