Please join us as Susan Whittaker, CPC, CPMA, Quality Improvement Program Manager at Maine Primary Care Association, presents our 3-part webinar series “Implementing Diabetes Programs.” Susan will be joined by Brenda Jenkins, Senior Program Administrator at Healthcentric Advisors, and Louise Ingraham, Patient Education/Navigation and Feargal Semple, Program Manager, both from the Maine CDC.
The 3 sessions will cover the following topics:
May 4 – Diabetes Self-Management Training (DSMT)
This session will outline DSMT services and how to be reimbursed for them. The program is designed to improve diabetes management, prevent co-conditions and slow the progression of diabetes type II.
May 11 – National Diabetes Prevention Program (NDPP)
Participation in an NDPP program is at no cost to patients with prediabetes. FQHCs will learn how to become an NDPP supplier and prevent diabetes type II. NDPP is a pre-requisite to becoming a supplier of the reimbursable MDPP program.
May 18 – Medicare Diabetes Prevention Program (MDPP)
The MDPP program is a 2-year Medicare reimbursable service for patients who have pre-diabetes or are at risk for developing diabetes. In this session FQHC’s will learn how to become an MDPP supplier.
This webinar series serves as an introduction to the Chronic Care Management ECHO series starting later this year.
