Health centers are increasingly focused on the need to identify and address the social determinants of health (SDOH) such as income, education, employment, housing, availability of food, and transportation.

Partly in response to changes in healthcare payment structures that focus on value over volume, healthcare organizations have begun to incorporate assessments of patients’ social needs and social risk factors into clinical workflows and have begun to develop processes and adopt technologies to refer patients to community based social services. As health centers continue to identify social determinants of health risk factors in their patients, many organizations are faced with deciding what information to share with community-based organizations to inform the patients care plan.
