Maine Mobile Health Program (MMHP) is hiring a Clinical Coordinator. Maine Mobile Health Program is a community health center focused on supporting Maine’s farmworkers and seafood processors in accessing healthcare. Position Summary: We are seeking a qualified candidate to assume the role of Clinical Coordinator (CC). The CC is responsible for supporting the clinical aspects of the Maine Mobile Health Program and ensuring that MMHP patients have full access to integrated health services during seasonal harvest periods. This position reports directly to the Executive Director and works closely with the Medical Director. This position supervises a nursing position as well as a medical assistant position.

 

What you will be doing:

Clinical Programming Support: Many clinical staff are part-time (primary care providers in the area or local family medicine residents), so the clinical director serves as a focal point for the organization.

·         Work with Executive Director and Medical Director on all ongoing clinical activities, projects, and programs to facilitate continuity of care.

·         Implement health education services, screening clinics, vaccination clinics, and other activities.

·         Assist with clinical updates and/or trainings for Community Health Workers (CHW’s).

·         Provide trainings for clinical providers with the goal of maintaining consistency in protocols and practice.

·         Stay up to date with clinical reporting systems and other clinical tracking.

·         Provide referrals for patients in the voucher fund program.

·         Maintain up to date records using the organization’s electronic medical record (must be comfortable with electronic record keeping and be willing to learn the “ins and outs” of the system).

·         Orient new providers to clinical workflows and electronic medical record (EMR).

·         Manage tracking and receipt of medical records flowing into the EMR; categorize documents, extract clinical data points to track Quality Measures for federal reporting, direct to appropriate clinicians for review.

·         Support annual Federal Uniform Data System (UDS) reporting.

·         Keep MMHP Mobile Unit Manuals updated.

·         Participate in Quality Assurance reviews of data and documentation; participate in QA Committee meetings.

·         Assist with designing and completing chart reviews to support quality improvement and chronic disease management (ie. voucher and provider audits, collection and analysis of chronic disease data).

·         Support MMHP’s Emergency Preparedness efforts, ensuring that all policies, plans, and equipment are up to date for emergency response.

·         Work collaboratively with the Behavioral Health Coordinator to ensure effective workflows for integrated Behavioral Health services.

·         Participate in MMHP trainings, retreats, and harvest debriefs.

·         Adhere to infection control/safety guidelines, confidentiality policies, personnel policies, rules, laws, and other regulations; stay up to date with CDC guidelines.

·         Provide support and advocacy during clinical emergencies (ie. COVID-19 outbreak at a farm camp).

 

Work Location:

The Clinical Coordinator will be based out of Augusta, ME.

What we will need from you:

·         The ideal candidate will have a background in nursing, public health, or health administration (licensure must be valid in the State of Maine and in good standing).

·         3-5 years of clinical experience preferred. It is also preferred but not required that this person have managerial experience.

·         Current driver’s license, with clean driving record for the past 3 years. Reliable transportation and insurance required.

·         Language abilities in the areas of Spanish and/or Haitian Creole is desired but not required.

·         Self-starter, independent learner, takes initiative.

·         Ability to organize, document, and prioritize deadlines as they pertain to reporting responsibilities.

·         Excellent computer skills is a must as most meetings are held remotely and coordination is often done through the electronic medical record

Pay: $62,400-$83,200/year

Expected hours: 40 per week
