Participants will explore evidence-based strategies for conducting empathic conversations and for connecting patients to appropriate internal and external supports\, while also supporting care team members in the screening process. The session will highlight best practices for integrating referral programs into existing or new internal initiatives\, as well as approaches to building more standardized\, sustainable workflows.
