DESCRIPTION:Quick Links:\nRegistration \nBe prepared for chronic care management coding changes that begin July 1st. \nCMS Medicare is phasing out the general chronic care management (CCM) code G0511 as of June 30\, 2025. FQHCs may begin billing CCM now with the base CPT codes for each service such as 99490 (as of January 1\, 2025); however\, FQHCs must use CPT base codes as of July 1\, 2025. The coding switch must be done on a chosen date for consistent billing for all Medicare patients.
