During this full-day, live learning retreat, Health Initiatives Consulting, a diverse team of industry experts will leverage decades of real-life health center experience and industry best practices to facilitate and engage attendees in this highly interactive, innovation and data-driven value-based transformation learning session. Attendees will learn:

• About the HCCN’s data-centric successes, challenges and the promising best practices that resulted in advanced care delivery innovations, along with how progress made can accelerate the advancement of the HCCN’s FY19 mission, strategic goals and data-centric innovation and value add success;    
• The impact and value the HCCN offers member organizations and how collaborative business and clinical integration strategies can be implemented to reduce population-centric total cost of care and facilitate more profitable VBP contracting; and
• Why investments in good data governance strategies and resources are so critical to whole-person care delivery and the overall health of an organization’s clinical and business success.
