SUMMARY OF FUNCTIONS: Responsible for providing comprehensive eye care to patients. This position reports to the Chief Medical Officer.
MAJOR DUTIES AND RESPONSIBILITIES:
Conduct complete vision exams.
Identify visual alertness, field of vision
Diagnose sight problems.
Assess patients for signs of disease.
Prescribe corrective lenses as appropriate.
Prescribe medications for ocular diseases and disorders.
Educate patients on diagnoses, products, and services to promote visual health.
Document services provided, prescriptions, treatment plans, and referrals.
ORGANIZATIONAL RELATIONSHIPS:
EXPERIENCE AND/OR EDUCATIONALREQUIREMENTS:
Graduate of accredited school of optometry (O.D.) with valid Maine license.
Minimum 2 years’ experience as Optometrist preferred.
Excellent communication, analytical, and interpersonal skills.
In depth knowledge of eye-related disorders and conditions.
PATIENT CENTERED MEDICAL HOME DUTIES:
Practices team-based care
Attends required staff meetings
Attends required training meetings
Participates in Health Center’s Quality Improvement processes
Is invested in fostering patient self-management including involving the PCMH Care Model.
Attend meetings at which Health Center, PCMH, and best-practice procedures and business agenda are addressed.
Attend Performance Improvement Team and other QI committees as assigned

POST-EMPLOYMENT REQUIREMENTS:
Successfully complete SCRFHC credentialing process and re-credentialing as necessary.
Maintain licensure in good standing during entire employment.
Complete mandatory training as assigned.
Maintain knowledge of best practices and innovations in Optometry through CME or other resources.
SCRFHC employment application required. Background check and credentialing process required.
