Maine Primary Care Association’s experience in the field of quality improvement, along with partnerships developed along the way, have served as a fundamental building blocks for our current efforts regarding primary care transformation towards the Patient-Centered Medical Home (PCMH) Model. In Maine the primary care medical home may be referred to in many different ways –  the patient centered medical home (PCMH), advanced primary care, or health home. In all these variations PCMH is a promising model for transforming the organization and delivery of primary care.

In a patient centered medical home, the patient is an active decision-maker and participant in their own health. The care team, rather than a single medical clinician, is responsible for working to address all aspects of a patient’s health.

Our intensive efforts to support this model are a natural progression in our ongoing efforts to transform and grow the role of primary care. Our organizational goal is that all Maine Community Health Centers will be recognized as providing patient centered care by 2015.
