Coding Changes in the FQHC for Chronic Care Management: Get Ready for a Smooth Transition

Be prepared for chronic care management coding changes that begin July 1^st.

CMS Medicare is phasing out the general chronic care management (CCM) code G0511 as of June 30, 2025. FQHCs may begin billing CCM now with the base CPT codes for each service such as 99490 (as of January 1, 2025); however, FQHCs must use CPT base codes as of July 1, 2025. The coding switch must be done on a chosen date for consistent billing for all Medicare patients. Join us to learn how to handle this transition and the advantages of the switch.
