Medicare’s Annual Wellness Visits are opportunities to get to know your patient and learn about their self-assessment of health and their personalized goals to include in their care plan.
Learn the flexibility within AWVs for performing an exam, satisfying quality measures and making referrals to needed services such as Community Health Workers, care management, dental services and diabetes prevention programs.
